Ramp Up Week

Ramp Up Week allows parents to fill out all the necessary paperwork to get each family ready for the 2022-2023 School Year from the comfort and convenience of their homes throughout the week of August 1st!

You will submit these documents online; however, if you need assistance, please don't hesitate to contact the school office by calling 765-447-2727, or email school@faithlafayette.org. We would be happy to help you.

Mandatory Forms

These forms are required for each student or household.

Give 3 Form

Faith Christian School provides many opportunities to serve. We encourage all families to volunteer in at least 3 areas or events. As families become involved in different aspects of the school, it is amazing to see the positive impact toward our family-oriented school culture. Please fill out the Give 3 Form to let us know where you would like to serve.

Food Allergy Form

Please fill out one Food Allergy Form per household.

iPad Agreement

This year all of our students will be using iPads! Elementary students' iPads will stay at school and be used in the classroom. Middle School and High School students' iPads will be issued at the start of school. High Students will have iPad Air devices. Please read through this iPad policy handbook with your student and complete the iPad agreement form with each student.

If you have questions, you can contact your student's principal or our Tech Coach, Kevin Kelso, kkelso@faithlafayette.org.

Elementary T-shirts (Elementary Only)

Please complete this form for each of your K5-5th grade students. These t-shirts will be worn on Faith Fridays and field trips throughout the year. The cost for each shirt will be billed to your school account.

Please note: Extra t-shirts will not be available for exchanges. This is a one-time order for the 2022-2023 school year.

Student Driver Permission Form

The administration at Faith Christian School views driving as a tremendous privilege for students. Our desire is for every driver to arrive and leave from Faith Christian School safely. In keeping with this goal, we need all student drivers and their parents to review and sign the Student Driver Permission Form.

Faith Community Center After School Permission Form (HS Only)
  • Faith Community Center After School Permission Form (Grades 9-12, if applicable)
  • This year, 6th graders who cannot be picked up after school will go to the elementary aftercare, and the family’s account will be charged accordingly. 7th and 8th graders may go to the after school detention room until 4:30pm if parents cannot pick them up after school. Any middle school students must be accompanied by their parent to be in the Community Center after school. 9th-12th graders may go to the Community Center after school upon completing the parent consent form during Ramp Up Week.
Carpool Opportunities

Some families have contacted us about the possibility of a carpool. Carpool could not only be a need for a ride for your student(s) to or from school, but even offering a space in your vehicle to take a student(s) to or from school. Please fill out the information at https://bit.ly/FCScarpool and we will get information to those of you who are also interested in a carpool option this school year.

FACTS Family Portal

Please take a few minutes to review and update family and student information. Log into the Family Portal, then

  • Click on Family
  • Click on Family Home
  • Click on the pencil icon to edit information
  • Money can be added to lunch accounts in the FACTS Family Portal by clicking the “Financial” tab and then “Add Funds” in the “Prepay Accounts” section.
New Drop Off and Pick Up Maps

We have also adjusted our Drop Off and Pick-Up procedures. We have shifted the parking towards Highway 26 for both the faculty and visitors. Parents of Middle School and High School students will then turn toward the school 1 row earlier. This will leave the far 2 rows of parking for students and enable them to enter the building without crossing traffic.
